The situation

What was actually going wrong

The clinic was generating leads at what looked like an acceptable cost per lead, but the sales team was drowning. Most enquiries were price shoppers, people outside the UAE, or contacts who never replied after the first message. Cost per lead looked fine on the dashboard. Cost per booked consultation was quietly terrible.

Separated intent instead of chasing volume

Campaigns were restructured around consultation intent rather than broad interest audiences. Treatment-specific ad sets replaced a single catch-all campaign, so the algorithm stopped optimising toward whoever clicked most cheaply.

Qualification moved into the form

Conditional questions were added to the lead form: treatment of interest, timeline, and whether the person was based in the UAE. Adding friction reduced raw lead volume and increased booked consultations, which is the trade every clinic should want.

WhatsApp became the closing channel

Click-to-WhatsApp campaigns ran alongside lead forms. In Dubai, patients qualify high-value decisions in conversation, and the WhatsApp route consistently produced better show-up rates than form fills.

Creative rebuilt around objections

New creative addressed the three questions that actually stop people booking: what it costs, whether they are a candidate, and what recovery looks like. Proof-led creative outperformed offer-led creative on every test.

Meaningful movement in lead quality appeared inside the first month once the campaign structure and form logic changed. The full 41% reduction stabilised over the following two months as creative testing compounded.

Raw lead volume fell initially because unqualified enquiries were being filtered out. Qualified enquiries and booked consultations both rose, which is the outcome the clinic was actually paying for.
